In children and infants there is a balance between high chest wall and low pulmonary compliance related to underdeveloped lung parenchyma, small airway diameter, and small alveoli (Tusman et al 2003).
Alveolar inflammation, lung tissue fibrosis, and collagen deposition contribute to diminished pulmonary function, including a decreased DLCO, and a decreased pulmonary compliance.
Also early during the onset of ARDS, pulmonary compliance decreases severely, contributing to an increased work of breathing and respiratory distress.
